To properly comprehend the aetiology of hemroids you need to have, at the very least, a basic understanding of the anal cushions. You see the anal submucosal venous plexus is somewhat unique in that it has discrete sacculations which intercommunicate with the arterial blood supply through a number of segments of veins of normal caliber.

These sacculations interact with the arterial supply in such a way that the juncture of their communication provides a mechanism through which they can become greatly engorged. Put simply these sacculations have the ability to greatly magnify the size of the anal cushions. This feature and its resulting ability to influence the size of the anal cushions does explain the unusually exuberant blood flow to the area.

How Hemroids Are Formed

The bottom line is that the anal submucosa is basically a specialized layer capable of tremendous changes in size  which when incorporated with the encircling internal and external sphincters provide a finely tuned mechanism in maintaining anal continence.

Normally during defecation the anal cushions are held out of the way by the anal submucosal muscle whose contraction flattens and braces them against the internal sphincter. First degree piles or hemroids are characterized by recurrent bright red blood which is most likely due to damaged capillaries within the lamina propria layer that is found immediately beneath the muscosal epithelium.

In the apparent absence of any discernible prolapse, the damage probably occurs as s result of the momentary displacement of the looser portions of the friable mucosal tissue during a bowel movement. The most likely chain of events is that those looser portions of the mucosal moiety are caught between the passing stool and the pectinate line (the place where the anal cushion is most firmly pinned down) resulting is some shearing damage that causes bleeding.

As the damage increases and as more mucosal tissue becomes involved this results in true prolapse where the mucosal tissue protrudes in various degrees within the rectum and anal canal.
